Policies & Insurance

At Elmbrook Family Dental, we believe you deserve the best care possible. We want to make quality care affordable for everyone. For this reason we have developed an extensive menu of financial options to help make a beautiful smile possible. We accept all traditional dental insurance plans and are Preferred Providers in most PPO's. We offer complimentary benefit analysis in order that you may see an estimated breakdown of your benefits. No insurance company really pays 100% of the bill.

ALL FEES AND INSURANCE CO-PAYMENTS ARE DUE AT THE TIME OF SERVICE UNLESS PRIOR FINANCIAL ARRANGEMENTS ARE MADE.

For payment we accept:

  • Cash, Check, Debit Card and AHC (auto withdrawal) payment from these accounts.
  • All major Credit Cards, Care Credit and CITI Healthcare.

 

Payment Options:

Full Payment at time of Service:

We offer a 10% courtesy allowance (12% for senior citizens – 62+ years) for treatment that is pre-paid in full (Cash, Check or Debit Card) when the appointment is scheduled. We also offer an 8% courtesy allowance (10% senior citizen discount) for payment at time of service. No additional discounts or discount plans will be honored in combination with this allowance.

Insurance Co-Pay:

We offer a 5% courtesy allowance for all treatment for which your estimated co-pay is paid in full at time of service. This allowance cannot be combined with any other discount or discount insurance plan. We accept most insurance and will file your claim for reimbursement.

In Office Financing with Approved Credit

  • Up to 3 months interest free with automatic debit from your Checking, Debit or Credit Card.
  • Up to 12 months interest free through Dental Banc with an initial down payment – auto withdrawal from your Checking or Savings Account.
  • We do not send statements. If a billing statement is required, we reserve the right to assess interest at 1% / month (12% annually).

 

External Financing with Approved Credit

    • 3,6 or 12 months interest free financing through Care Credit or CITI Healthcare.
    • Up to 60 month financing is also available to provide lower monthly payments.
